On receipt of this form Vonage will respond within 2 business days. Termination of your Vonage Service will occur on the date you have requested provided you have told us that you do not wish to listen to offers and have indicated that you have understood the important information at the bottom of this form below. As set out in section 3.2 of our Terms of Service, disconnection fees including other outstanding charges may be charged when you terminate. You will receive confirmation of your termination by email.
